Simply Medicine (Simply Medicine OK) is a cash-pay telemedicine practice serving patients in Oklahoma, operated by Jeffrey L. Halford PLLC. This page explains how we handle personal information we collect through our website (simplymedicineok.com) and through Facebook / Instagram lead forms.

When you submit a Facebook or Instagram lead form for Simply Medicine, we may collect:
We do not ask for diagnoses, symptoms, medications, insurance details, or other health information on Facebook lead forms. Please do not put medical details in a lead-form message field.
If you book or message us through our website or patient portal, we also collect the information you provide there (for example contact details and the clinical information needed for care).
We use lead-form contact information only to reach you about Simply Medicine care — for example to answer your inquiry, help you book a visit, or follow up about scheduling. We do not use lead-form data for unrelated marketing lists.
We do not sell your name, email, or phone number. We do not share lead contact information with third parties for their marketing. Meta (Facebook/Instagram) delivers the lead to us under Meta’s terms; once we receive it, we use it as described here.
We may use service providers that help us operate the practice (for example our patient portal / EHR and email or phone tools). Those providers process information only to support our services.
Our website may use the Meta Pixel and Google Ads tag to measure visits and ad performance. Those tools can collect device and browsing data according to Meta’s and Google’s own policies. We do not intentionally send health information through advertising pixels.
We keep lead contact information only as long as needed to respond to your inquiry and operate the practice, or as required by law. You can ask us to delete a lead record that has not become part of a clinical chart.
Email or call us to update or delete your lead contact information, or to ask how we used it. If you become a patient, additional rights and notices apply to your medical record under applicable health privacy law.
